Criminal background check For a parent or guardian who would like to volunteer to help chaperone. This must be filled out, submitted, and approved before a parent or guardian may chaperone any school sponsored event.

All students participating in marching must fill out the band pre-physical form questions page 1 above each year in band. New students to the district, Freshman and Juniors must get a completed physical exam by a medical professional. Also, if a student answers yes to any question 1-6 in the pre-physical they must also complete a complete physical exam by a physician. If a student got a physical for athletics/dance/cheer or any other activity a copy needs to be supplied to the band staff.

This form must be signed and returned to JHS band
Medical Release Form Required for all students in band, jazz band, winterguard, and winter drumline. This form must be Notarized, the counselors office at JHS has a notary.
